Payment & Insurance Information

Lake Michigan Dental wants your overall experience with us to be positive. Our caring staff understands that dealing with dental insurance can often be confusing, frustrating, and unpleasant.

Our office will answer any questions you may have regarding your options for payment for your dental services.

We also offer CareCredit as an option for long-term payment plans, although this just be arranged prior to any services being performed.

Note: All copays are due at the time of service.

Dental Insurance

As of January 1, 2023 Lake Michigan Dental will be Out-of-network with ALL Delta Dental insurance plans.  As an Out-of-Network Delta Dental provider we will continue to accept Delta Dental insurance and file your claims for you. Lake Michigan Dental accepts payment from most dental and health insurance providers. All Delta Dental payments along with Blue Cross/Blue Shield payments go directly to patients and not our office, so payment in full is requested at time of service. This payment arrangement is due to how your employer set up your dental insurance plan with Out-of-Network claims submissions.

Please take the time to read through your benefit information so that you are familiar with all of your policy’s benefits and limitations. We also encourage you to contact your insurance company if you have questions about whether a service is a covered benefit, keeping in mind that they will never guarantee payment.

As a courtesy, we will bill your dental insurance provider directly. Please keep in mind, however, that your insurance policy is a contract between you and your insurance company. Since we are not an involved party with this contract, you are responsible for any non-payment on our account. We also require that all copays and deductibles be paid at the time of service.

Please remember to let us know at the beginning of each visit if there are any updates or changes to your insurance policy or your employment status. That way, we can assist you in maximizing your benefits.
